Making A Referral
Support Brokers
The ultimate aim of Support Brokers is to see people taking full control of their own lives.
Support Brokers are independent and work alongside people, supporting them to plan how to live in the way they choose, a life that makes sense to them.
The Support Broker will encourage people to think about what they really want and find ways to work towards their perfect life. They work directly for the person; supporting them to stay in control, to understand and explore a full range of opportunities and to become confident to take the action needed to achieve their desired outcomes.
Listed below are some of the functions of a Support Broker:
- Provide information, advice and guidance relating to health and social care processes and community opportunities.
- Signpost people to the information they need, following up to check on the outcomes.
- Assist in the development of a Person-Centred Plan.
- Identify a range of opportunities and resources within a person’s community, assist the person to find ways to access these and to make connections.
- Work alongside the person and their network of support, or assist them to build a good network of support.
- Supporting the person (and/or their family) to communicate and negotiate with service and support provider organisations
- Support to implement the Person-Centred Plan, assisting to support Support Workers.

Person-Centred Planning
Person Centred Plans capture what is important in a young person’s life, what is working well for them and the areas of their life that they would like to change.
At Beams, we work alongside people to facilitate conversations that gather the detail for their plan, ensuring it clearly expresses the areas of their life they wish to change and what they wish to achieve. The Plan is what helps people to stay in control and be in the driving seat of their life; and it helps everyone else to understand how that should happen.
An individualised Person-Centred Plan can start life as a blank sheet of paper. The information builds over time until the important details have been captured that truly represent the person’s wishes, including future direction, skills and interests.
The content of a Person-Centred Plan includes:
- what is important
- what needs to change and/or be achieved
- what is working
- what is not working
- how the person will stay in control
- who will make important decisions
- how any risk or potential risk will be reduced / managed and what contingency plans will be put in place
- how a Direct Payment, Personal Health Budget or personal budget can be used
- how the plan will be put into action after it has been approved
Reviewing Progress of Your Support Planning
We can support you with reviewing the of progress of your Support or Person-Centred Plan which helps to keep things on track, by holding regular review meetings.
The frequency of the review meetings will be up to each individual and this can change depending on what is happening in their life at any particular time.

Brokerage Management
Brokerage Management can help your young person receive and manage their Direct Payment support; we can help find PA’s, manage and monitor staffing, legal requirements and paperwork.
At Beams, we offer Brokerage Management services to help people to manage their Personal Budget or Personal Health Budget or Direct Payment, including assistance with recruiting to Personal Assistants/Support and Care Workers.
Some people will need ongoing support with the implementation, management and administration of their individual Support Plan arrangements, these functions are called ‘Brokerage Management’.
Brokerage Management includes:
- Management of your Personal Budget/Personal Health Budget/Direct Payment
- Assist with recruiting Support Workers/ Personal Assistants including DBS checking
- Management and development of staff
- Planning, running & overseeing team meetings for Support Workers/Personal Assistants
- Facilitating ‘Circle of Support’ meetings
- Payroll function
- Arranging insurance
- Exploring & applying for funding to help cover the cost of your arrangements
- Reviewing and updating your Support plan as needed
- Setting up and managing your direct payment

Making A Referral
Person-Centred Planning
Requests for Person-Centred Planning can be made directly by the client, or on their behalf by professionals, family members or other involved parties. Where a referral is made on behalf of a client, the client should have agreed to the referral being made prior to sending this to the Brokerage Service.
Brokerage Management
Where you are directly funding management support for your personal budget you can make a request for support on our referral form.
If the management support is being funded by the Local authority, your Social Worker or Direct Payment Case Worker will make a referral on your behalf.
